Dr. Wolfgang Schaden is an Austrian trauma surgeon and a founding figure in the development of medical shockwave therapy, co-founding the International Society for Medical Shockwave Treatment (ISMST) and the German Society for Medical Shockwave Treatment (DIGEST).

Over several decades, his work has helped expand shockwave-based therapies from lithotripsy into musculoskeletal and regenerative applications, including delayed and non-healing fractures. That combination of active trauma-surgical practice and decades of shockwave research gives his perspective on non-union management a specificity that comes from having watched the technology evolve from within both disciplines.


What role can shockwave therapy play in bone healing?

Delayed and non-healing fractures — non-unions — particularly in the lower extremities, can significantly affect a patient's mobility, quality of life, and ability to work, Dr. Schaden explained. Conventional surgical treatment is often complex, sometimes involving long operating times, multiple surgical sites, and risks such as infection.

Extracorporeal shockwave therapy (ESWT) is a non-invasive approach used in certain cases to support biological healing processes. Clinical experience and published studies suggest that, in appropriately selected patients, shockwave therapy can stimulate bone regeneration and may help patients avoid or delay surgical intervention; reported healing rates in the literature are described as meaningful in many patients. Treatment success, Dr. Schaden noted, depends on multiple factors — including the specific indication, the patient's condition, and the clinical protocol used — and a thorough medical evaluation is essential to determine suitability.


What distinguishes electrohydraulic shockwave technology?

Electrohydraulic shockwave systems, Dr. Schaden noted, are characterized by their ability to generate high-energy acoustic waves with broad tissue penetration. In his clinical experience, this can allow effective treatment with fewer sessions than some alternative technologies; protocols vary, but some practitioners report achieving therapeutic effects within one to two sessions in selected indications.

Fewer treatment sessions can reduce the overall treatment burden for patients and may improve integration into daily life, he added. The optimal treatment approach, however, should always be determined by a qualified healthcare professional based on individual patient needs.


What future developments are expected in shockwave therapy?

Dr. Schaden considers shockwave therapy a field with substantial ongoing research potential. Alongside its established musculoskeletal indications, he pointed to current investigations into its possible role in regenerative and immunomodulatory processes, including:

  • chronic wound healing;

  • nerve regeneration;

  • lymphatic disorders;

  • spinal cord injury;

  • other regenerative applications.

Early-stage studies are also examining potential interactions with the immune system, he noted — applications that remain investigational and are not yet part of routine clinical practice. Looking ahead, Dr. Schaden anticipates that regenerative treatment approaches will play an increasingly important role in medicine, though he stressed that further clinical studies are required to confirm safety, efficacy, and appropriate indications.
